Yes, "tootsie pie" is very much like "honey" or "sweetie pie."

It was used when I was young.

I'd never heard that one. "Sweetie pie" is a term of endearment, as is "honey pie". If he asked "Can I see a picture of *your* (and not 'you') tootsie pie", there's a chance he was alluding to something vulgar, but let's give him the benefit of the doubt. Personne ne m'appelle ainsi, c'est curieux...
